Alabama Lens Program for VISION USA Patients

Optometry Cares – The AOA Foundation is proud to partner with the Essilor Vision Foundation (EVF) to offer lens and lab services at no cost to optometrists providing services for VISION USA patients using EVF’s Online VisionWeb system.

ESSILOR VISIONWEB STEP-BY-STEP INSTRUCTIONS

How to place a lens order through VisionWeb:

  1. Navigate to visionweb.com
  2. Log-In using your VISION USA credentials – do not use any other credentials you may already have set up in VisionWeb.
  • Sample Username: ALVU555555 (ALVU+6 digit AOA Member ID Number) ID Number can be found in the address field of the patient assignment letter mailed to you for each assignment (if unable to locate AOA Member ID Number contact VISION USA at 314.983.4261 for assistance).
  • Password: vision (all lower case)
  1. Double – Click “Spectacle Lens Order”
  2. Enter your order using the information on your exam form or RX form
  • Job Type:
    • Frame to Follow – send the frame to the lab with the printed VisionWeb order. The lab will edge and mount the lenses in the frame and send the completed job back to you.
    • Uncut – orders an uncut lens. Lab will immediately begin surfacing the lens and will send it back uncut.
  • Lens Type: This field will default to Single Vision unless you have entered an “Add Power” in the RX fields. In which case, it will default to BiFocal.
  • Lens Design:
    • Single Vision Jobs, select “Single Vision” unless an aspheric lens is specifically requested by the signing doctor.
    • BiFocals, Flat Top 28 will default.
  • PO Number: Field is not required. You can indicate Vision USA or leave blank.
  • Frames Information: Field not required. Recommend completing to assist lab in frame verification.
  • Lens Material:
    • Use the chart to choose lens material.  For biFocals, use Litestyle if the chart recommends Polycarbonate. (ignore this chart if doctor has requested a specific lens).  Note:  use the eye with the highest power RX (sph or cyl) to select lens material
  • Click “Send Order Now” and Print Copy of the Order 
    • If ordering “Frame to Come” jobs, wrap one copy around the frame and place in a zip-lock bag or rubber band. This should be sent to Southern Optical lab as soon as possible:
    • Southern Optical
    • ATTN: Frame to Come
    • 136 Industrial Park Rd #B
    • Piney Flats, TN 37686
    • Note: If the lab has not received the frame within 5 business days, the order will be canceled.
  • If ordering an “Uncut” lens job, you are done. Print a copy of the order detail for your records. 

IMPORTANT INFORMATION:

  • Mail or Fax yellow copy of Patient Information Form to VISION USA, 243 N LINDBERGH BLVD, ST LOUIS, MO 63141 or 314-991-4101 for reporting purposes and (1) copy is kept for the provider’s files.

  • Assignment of your VISION USA patient, contact VISION USA.
  • Set up a VisionWeb account or if you have an issue with your order, contact Essilor Vision Foundation.
  • State program, contact Alabama VISION USA Coordinator.
